Child abuse is the leading cause of death in young children, accounting for 1-4% of the general hospitalizations. The purpose of this study was to develop a comprehensive data bank on the forensic and physical findings of child abuse victims in order to highlight the importance of recognizing these injuries in children. The medical records of all patients with the diagnoses of: "Crush", "Sudden Infant Death", "Shaken Infant Syndrome" and "Angle Child Trauma" were retrospectively reviewed for the presence of any injury to the upper extremity. During a 5-year period, 12 cases of upper extremity injury due to child abuse were noted. Nine children were male and 3 female. Median age was 1.5 years (range: 2 mos-7 yr). Eleven of the 12 children had bilateral injuries of the upper extremity. Of the 12 cases, 9 children had injuries to the forearm, 3 children had injuries to the hand and 2 children had injuries to the shoulder girdle. In one child, the trunk was the only site of injury. All cases were associated with significant multisystem injuries. All children except one were injured in the home. The high incidence of bilateral upper extremity injuries in abused children is thought to be due to the use of the upper extremities as a self-defense mechanism by the abuser. As most of these children were injured at home, they presented to the emergency department with little to no complaint. Most of the abused children with upper extremity injuries have significant associated injuries with the likelihood of a multisystem trauma. Upper extremity injuries are usually bilateral and may appear in the context of multisystem injuries.
